Public Bus Consent Form

Completion of the below form provides consent to Birali Steiner School Staff to allow your student/s on to the afternoon bus service as well as provide advice for supervision purposes of student/s likely to be arriving on the morning bus services.
A staff member from Birali will mark off each student who boards the bus, if consent has been provided. Please ensure that your student/s are aware each day if they are expected to take the bus that afternoon. The staff member on Bus Duty will have a list of which students have consent to take the bus (not specific days). Please remind your child at drop off if they are expected to take the bus that afternoon. Students will be unable to board the bus unless the school has received the appropriate consent signed by a parent or guardian.
